Dr. Handshuh and our team are happy to help you better your oral health and welcome you to the practice. Before your first appointment, we ask that you please print the new patient forms, which are located in the link below. Please fill out the paperwork to the best of your ability, and bring it with you to your first exam. If you are unable to print the forms, just let us know! We will gladly mail them directly to you, for your convenience.
